We look to welcome each new patient into our practice as if you were a member of our own family. Your first visit to Five Points Family Dental involves a few special steps so that we can get to know you. We are not in-network with any dental insurance companies; however, most plans include out-of-network benefits, which enables you to choose the dentist you want with the possibility of not incurring any out-of-pocket expense. We will file the claims for you and have the payment sent to us if your plan is set up that way.
Once you schedule an appointment, you will receive an email and text message providing you with a link to fill out our paperwork ahead of your arrival. Please arrive at least 15 minutes early for your appointment in order to address any outstanding paperwork or financial considerations. Please bring a copy of your insurance card. If you have recent X-rays from another dental office, please call us for our email address so you can send them in advance of your appointment.







What To Expect
During your new patient exam, we begin by using a small intra-oral camera to take high-resolution pictures of your teeth, followed by X-rays of your teeth and jaws. Dr. Nelms will do a thorough examination, checking your head, neck, joints, gums, and teeth for any problems. We will review any findings with you and use the X-rays and photos to show you what we see. You will receive a custom treatment plan presented to you in an easy-to-understand and sequenced phasing of appointments, along with any alternatives or options you may have based on your case. You will also have a financial consultation to explain any fees associated with the care and what your insurance will cover.
Dr. Nelms believes that taking the time to educate patients empowers them to prioritize their own health and well-being. We meet each new patient where they are because it’s never too late to begin a journey toward oral health.

Our office is located in the heart of Five Points at 1761 South Lumpkin Street, Athens, GA. We have our own parking lot with an ADA-accessible entrance and parking space available.
